Complete the intake process and setup your Patient Account and pay initial fees.
- Review Privacy Notice
- Read and sign Policy Agreement and Consent to Treatment Form
- Complete Release of Information form and other intake documents
- Schedule the initial sessions (number of sessions will be based on your treatment plan)

My journey towards addiction counseling and focus on mental health disorders begun with my love for youth, and as a PreK-12 educator, I am committed to the health and well-being for the whole family.
I am Independent
As a private practice independent from any treatment center or health management systems, I focus our attention on meeting your needs and I walk with you until you are ready to walk on your own.
